Using Our Template Questionnaire
We have prepared a template questionnaire response for the government consultation to make it simpler to send in a response, even without the time to create one from scratch. Obviously, it would be better if you personalised it by checking through and adding your own thoughts, (or changing anything you don’t agree with of course!).
right click on this link (and select “Save”) and save the file to your hard disk. Then use your email program to email the file you’ve just saved, as an attachment to :
family.consultation@hmcourts-service.gsi.gov.uk (click to open your email program)
You can optionally also tell the DCA who you are, if you want to do this, also save and download this file at the same time, edit it in Word to put your personal details in and then send it in the same email as a second attachment to the above email address.
It is probably best to send this “About You” document as well as then they will send you an acknowledgment when they receive your questionnaire. It’s up to you.
